Resident Assistance Animal Declaration
Version 1.0 · October 7, 2026
In this declaration, "the Unit" means Unit {{Unit number}} at {{Property name}}, and "Applicable Leases" means each lease agreement for the Unit, including any animal addendum, in effect when I sign or entered into at renewal.
You do not have to use this form. You may ask for an assistance animal accommodation at any time, orally or in writing, and it will be processed the same way (Fla. Stat. § 760.27(3)(b)). There is no charge to submit or renew this request.
This is the declaration for every assistance animal in the Unit, whether or not a pet also lives there. I sign it before giving any animal details, then complete one PawFred Animal Profile for each assistance animal (Part 2). If a pet also lives in the Unit, it is registered under its own declaration (the Pet Resident Declaration (Form RPD) for a dog or cat, or the Other Animal Declaration (Form ROAD) for any other pet) which I sign before this one. Each type of animal has its own declaration, and each animal is linked to the declaration for its type. The declarations stand alongside one another, and none of them replaces or limits another. Nothing in a pet declaration applies to an animal registered under this declaration, and signing or not signing a pet declaration does not affect this request.
In Parts 3 to 6, "the animal" means each assistance animal registered under this declaration.
1. Service animals and emotional support animals
A service animal is a dog or miniature horse individually trained to do work or perform tasks for a person with a disability (Fla. Stat. § 413.08(1)(d)).
An emotional support animal is an animal that provides support that alleviates a symptom or effect of a disability and does not require training (Fla. Stat. § 760.27(1)(a)).
2. Animal Profiles and supporting information (submitted after this declaration)
After signing, I will complete one Animal Profile in PawFred for each animal, including any supporting information requested during verification.
Do not include a diagnosis, its severity, or medical records; you may share them only if you choose to (§ 760.27(3)(a)). A certificate, ID card, patch or registration bought online is not enough on its own (§ 760.27(3)(c)).
Incorporation by reference. Each Animal Profile, every document submitted with it, and every later update carries a PawFred record ID linked to this declaration. Each is incorporated by reference into this declaration and forms part of it as if set out here in full. The representation and warranty in Part 6 applies to each one on the date it is submitted.
3. Verification and decision
PawFred will verify each Animal Profile under the assistance animal procedures in {{Property name}}'s Animal Policy and applicable law, including the Fair Housing Act, Fla. Stat. § 760.27 and § 413.08, and may ask for more information. PawFred's verification is not a decision: all final determinations on the registration and approval of any assistance animal belong to {{Property name}} only, and {{Property name}} gives its decision in writing. Rules that apply only to pets, such as breed, size, weight and number limits, pet-free areas and pet-specific insurance, do not apply to an approved assistance animal; any safety concern is assessed based on the animal's own conduct.
Completing registration and verification. I acknowledge that a request made through PawFred is received and processed when I complete registration, including an Animal Profile for each animal. There is no payment step. I agree to take all steps needed to complete verification of each animal, and to provide requested information and answer each follow-up request within 7 days of the request, or to ask for more time if I need it. This does not limit my right to request an accommodation in another way (Fla. Stat. § 760.27(3)(b)).
4. What I agree to
I will follow {{Property name}}'s community rules on animal conduct, which apply to every animal at {{Property name}}, including assistance animals: leash rules, waste pickup, noise and common-area conduct, and keeping the animal under my control. A leash is not required if my disability prevents its use or a leash would interfere with a service animal's work; the animal must then stay under my control by voice, signal or other effective means.
I am responsible for any damage or injury the animal causes to the premises or to any person on the premises, as provided by Fla. Stat. § 760.27(4) and § 413.08(6)(b), the Applicable Leases and Florida law, including Fla. Stat. § 767.01 for dogs.
The animal must be housebroken and under my control at all times.
Only rules that apply to pets as pets do not apply to an approved assistance animal: pet fees, pet deposits and pet rent; breed, size, weight and number limits; rules that keep pets out of common areas residents use; and pet-specific insurance requirements (§ 760.27(2); § 413.08(6)(b)).
Approval may be withdrawn if this specific animal poses a direct threat to health, safety or property that cannot be reduced by another reasonable accommodation (§ 760.27(2)(a)).
I will update the Animal Profile through PawFred within 10 days if the animal leaves the household or the need ends.
5. Privacy
Information in this declaration and each Animal Profile is used only to review this request and carry out any accommodation. PawFred stores it securely, limits access to PawFred's verification team and {{Property name}}'s decision-maker, and protects it as required by Fla. Stat. § 501.171. It is kept apart from the general lease file and is never shared with other residents. Other staff see only that a registered animal lives in the Unit, not its type, and PawFred tags look the same for every animal.
6. Representation and warranty
I represent and warrant that the information in this declaration, in each Animal Profile I submit under it, and in every document submitted with them is true, complete, accurate and genuine on the date I sign this declaration and on the date each Animal Profile is submitted or updated.
This declaration, including each Animal Profile incorporated into it, is incorporated into and forms part of the Applicable Leases. A false or misleading statement in this declaration or any Animal Profile, or a falsified document, is a material noncompliance with the Applicable Leases. {{Property name}} may then withdraw the accommodation, apply its Animal Policy terms for pets to the animal from the date of the misrepresentation, and use every remedy available under the Applicable Leases and Florida law, including Fla. Stat. § 83.56(2).
Florida notice. It is a crime in Florida to lie about needing an assistance animal. Anyone who knowingly gives false information or documents to get an emotional support animal approved, or knowingly and willfully pretends to have a disability or disability-related need for one, commits a second-degree misdemeanor (Fla. Stat. § 817.265). The same is true for anyone who knowingly and willfully pretends to use, or to qualify for, a service animal (Fla. Stat. § 413.08(9)). Each offense is punishable by up to 60 days in jail, a fine of up to $500, and 30 hours of community service.
7. Signature
I sign this declaration for the Unit under the Applicable Leases, on behalf of everyone who lives in the Unit, and represent that I am authorized to do so. I agree that my electronic signature has the same effect as a handwritten signature (Fla. Stat. § 668.50).
